29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a.)  On or about February 24, 2022 at the worksite, two employees conducted masonry work while on a scaffold that was not fully planked between the front uprights, therefore exposing employees to a 9 foot 6 inch fall hazard.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms.  a.) On or about February 24, 2022 at the worksite, employees doing overhead bricklaying from the top level of the scaffold were exposed to an approximate 32 foot fall hazards, in that, the scaffold was missing guardrails on ends of the scaffold sides.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.)	On or about February 24, 2022, at the worksite, employees engaged in masonry work on the third level were exposed to a fall hazard of 22 feet- 8 inches, in that, fall protection was not used.
