Establishment - Saco Valley Branch: The Employer did not ensure walking-working surfaces were maintained free of tripping hazards in the following instances: a. Rear door - missing and loose concrete creating a 2mm deep recession. b. Sunken drains - aisle way between washers 58 and 49 (2mm deep), washers 48 and 39 (1mm deep), washers 35 and w27 (>1/8 inch deep). c. Missing floor tiles near folding tables. d. Back room (across from restroom) - drain hole without cover resulting in a 6 inch diameter hole, 1.5 inches in depth and reducing to a 2.5 inch diameter hole that is an additional 5 inches deep.
