Location: a) Fourth Floor, by stairway  a) On or about 03/04/2022, an employee left his work area to take a lunch break and did not turn off the equipment. The arc welders were not turned off at the source.

Location: a) Fourth Floor, by stairway  a) On or about 03/04/2022, an employee was welding a steel frame next to the exposed edge of the fourth floor, by the stairway, without the use of fall protection and was exposed to falling 8 feet to the stairway landing below.
