29 CFR  1910.217(c)(3)(i)(b): The employer did not provide and ensure guarding prevented the employer from inadvertently reaching into the point of operation, or withdrawing his hands if they are inadvertently located in the point of operation as the dies close:   a) At the establishment, on or about 3/2/2022: The employer permitted use of an AccuPress, Serial Number 8385, without ensuring protection against point of operation.   Metal Logix Design and Fabrication Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.217(c)(3)(i)(b), which was contained in OSHA inspection number 1399412, citation number 1, item number 3b and was affirmed as a final order on 5/6/2020, with respect to a workplace located at 1305 Conklin Avenue, Utica, NY 13501.
