29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about March 1,2022; Roof area; employees were operating tools such as but not limited to pneumatic nails guns without eye protection while conducting roofing activities.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about March 1, 2022:  Employees  working working on the roof approximately 35-feet from the ground level were wearing  improper fall protection exposing them to injury in the event of a fall.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about March 1, 2022:  Where employees were observed working at an elevation of 35 feet without fall protection the employer did not certify that fall protection training had been accomplished.   In accordance with 29 CFR 1903.19(d), abatement ce1tification is required for this violation (using the Certification of Corrective Action Worksheet)
