29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.   Worksite: Prior to workers being  assigned to use the Toyota forklift, the employer did not ensure that each operator had been evaluated as required by 29 CFR 1910.178(l).

29 CFR  1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded:  1) Storage and Parts Area: The electrical panel, located adjacent to racks, was blocked with storage of materials; 2) Building and Testing Area: The electrical shutoff to equipment in the area was blocked by storage of construction materials.
