29 CFR  1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.  a) Location - 193 Lions Mouth Road, Amesbury, MA  On or about 03/11/22,  an employee was observed creating puddle welds with cables that had repairs (with electrical tape) in the first 10 feet from the electrode holder.

29 CFR  1926.760(c)(3): The boundaries of a Controlled Decking Zone (CDZ) were more than 90 feet (27.4m) wide and 90 (27.4 m) feet deep from any leading edge.  a) Location - 193 Lions Mouth Road, Amesbury, MA  On or about 03/11/22, employees were exposed to fall hazards of 15 feet 4 inches while working in a CDZ that exceeded the maximum allowable dimensions.  The established CDZ was 120 feet wide by 115 feet deep.
