29 CFR  1910.23(b)(10): (10) Any ladder with structural or other defects is immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced;   (a) Shop - On or about March 11, 2022, a 6-foot Werner folding ladder with a bent horizontal member, and a defective spreader bar was not removed from service, exposing employees to a fall hazard.

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  (a) Facility - On or about February 24, 2022, the employer did not report a hospitalization that the employee suffered as a result of work-related injury until March 10, 2022.
