Employees were thereby exposed to hazards associated with slips and falls.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a. Warehouse - On or about March 16, 2022, in the facility, employees Southwest and Southeast exit doors were obstructed by material which interferers in emergency evacuations. Thereby exposing employees to hazards associated with fire or other  emergency evacuation hazards.

Warehouse - On or about March 16, 2022, in the facility, employees were required to operate a Kalmar Forklift Model 060BXHPSN253C on a daily basis. This powered industrial vehicle was operated with a condition that affected the operators safety, thereby exposing employees to struck-by hazards.
