29 CFR  1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that: 1) is work-related; and 2) is a new case; and 3) Meets one or more of the general recording criteria of 29 CFR 1904.7 or the application to specific cases of 29 CFR 1904.8 through 1904.12:  On or about March 14, 2022, at an establishment located at 3456 W. College Avenue, Appleton , Wisconsin, the employer had not recorded an injury that occurred on March 6, 2022 and resulted in the employee not working on March 7, 2022.

College Avenue, Appleton, Wisconsin, the employer had not created an annual summary of the 2021 OSHA 300 Log and posted the annual summary by February 1, 2022 in the workplace.

29 CFR  1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  On or about March 14, 2022, at an establishment located at 3456 W. College Avenue, Appleton , Wisconsin, employees placing frozen beef, chicken and pork slices from boxes into Cambros were not provided appropriate hand protection for working with cold products.
