29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly, and in a sanitary condition: (a) On or about 03/16/2022 - Grinding department, employees loading scrap materials onto a conveyor were exposed to slip and fall hazards due to excessive amounts of scrap plastic being allowed to accumulate in the walkways of the scrap grinder. (b) On or about 03/16/2022 - Throughout the facility, employees were exposed to slip and fall hazards due to water accumulation from the roof leaking.
