On a 7/12 pitched residential roof located at 11162 Southwest 34th Road, Lot 328 in Gainesville Florida: On or about March 17, 2022, the employer exposed employees to fall hazards ranging from 6 to16-foot, in that fall protection was not utilized by employees, while performing roofing work .

On a 7/12 pitched residential roof located at 11162 Southwest 34th Road, Lot 328 in Gainesville Florida: On or about March 17, 2022, the employer exposed employees to fall hazards ranging from 6 to 10-foot, in that the portable ladder used to access the roof was not extended at least 3-foot  above the upper landing surface.

On a 7/12 pitched residential roof located at  11162 Southwest 34th Road, Lot 328 in Gainesville Florida: On or about March 17, 2022 the employer exposed employees fall hazards ranging from 6 to 10-foot, in that the portable ladder used to access the roof was not set up properly in that the angle was not one-quarter of the working length of the ladder.
