29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    228 Hartford Turnpike, Shrewsbury, MA: On March 17, 2022 and at times prior, the employer did not maintain effective frequent and regular inspections by a competent person of the personal fall protection equipment that was provided for employee and subcontractor use at the 228 Hartford Turnpike, Shrewsbury, MA site. Specifically, on March 17, 2022 and at times prior, a Guardian Aardvark Self-Retracting Lifeline device and other fall protection equipment was made available to employees at the jobsite that had not been not  been regularly inspected for wear, damage, corrosion and or other defects by a competent person.
