These hazards include but may not be limited to falls of approximately 20 feet while using an extension ladder that was not positioned three rungs above the eave to access the roof of a two story building.   b) Building 6, 4th and Olive, Idaho Falls, ID 83404: on or about March 15, 2022; the controlling employer, Roth Properties, Inc. did not have an effective system in place to correct hazards to which their subcontractors employees were exposed to. These hazards include but may not be limited to fall hazards of approximately 10.7 feet while performing framing activities without fall protection.   c) Building 4, 4th and Olive, Idaho Falls, ID 83401: On or about March 15, 2022; the controlling employer, Roth Properties, Inc. did not have an effective system in place to correct hazards to which their subcontractors employees were exposed to. These hazards include but may not be limited to fall hazards of approximately 9.3 feet while performing roofing activities on a 5:12 sloped roof without fall protection.  Note - Abatement certification and documentation is required for this item.
