29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about October 1, 2021, an employee was working on the roof and he was tied off to his safety harness at a point on his waist area.  The fall hazard ranged from about 10 to 20 feet.  The pitch of the roof was about 7 in 12.

29 CFR  1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  (a)  Jobsite:  On or about October 2, 2021, an employee was working from a metal ladder9extension type) and the ladder was not secured.  This ladder collapsed while the employee was working from it and the employee was severely injured as a result of the fall.

(a)  Jobsite:  On or about October 2, 2021, the employer did not have a training program on ladders that would enable each employee to recognize hazards related to  the use of ladders and minimize those hazards.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  (a)  Jobsite:  On or about October 2, 2021, the employer did not report to OSHA an injury that happened on the jobsite that resulted in the hospitalization of an employee.  The employee suffered sever head injuries, loss of sight in one eye and the loss of hearing in one ear as a result of a fall.
