29 CFR  1910.132(d)(1)(i):When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard:  Warehouse: On or about 12/6/2021, and at times prior, the employer exposed employees to corrosive hazards in that caustic chemicals such as 66185, caused chemical burns to the feet while employees were required to clean up spills with steel toe boots as required personal protective equipment.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Warehouse: On or about 12/6/2021, the employer did not report to OSHA within 24 hours the hospitalization of an employee resulting from a work related injury. The injury occurred on 12/6/2021 but the employer did not report the injury until 2/10/2022.
