29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) On or about March 21, 2022, at the worksite located at 1274 Bell Ave., Hartford, WI, the employer did not ensure step ladders were used for the purpose they were designed. Workers were using a step ladder in the closed position, and leaned against a wall, in order to perform work, thereby exposing employees to fall hazards.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  (a) On or about March 21, 2022, at the worksite located at 1274 Bell Ave., Hartford, WI, the employer did not ensure all personnel received training in safe ladder usage. Workers were using a step ladder in the closed position, and leaned against a wall, in order to perform work, thereby exposing workers to fall hazards.
