29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 23, 2022, an employee was exposed to flying metal particles while operating the "Norton" (No serial number, identified as Grinder #2) outer-diameter (OD) grinder.

29 CFR  1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage.  On or about March 23, 2022, at the Landis OD grinder, an employee was exposed to an electrical hazard while using a 120v lamp that did not have a cover to protect he bulb.

29 CFR  1910.305(g)(1)(iv): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  On or about March 23, 2022, in the shop area, employees were exposed to electrical hazards from extension cords being used as a substitute for the fixed wiring.
