29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At a worksite located at 1237 S Rangline Rd, Joplin, MO 64801: Employees were exposed to struck-by hazards and were not provided with the proper personal protective equipment  On or about February 8, 2022, decking was being added to a wood-framed building. Employees were using a pneumatic nail gun to secure the decking to the structure and were not wearing protective eyewear while using the nail gun.

29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  At a worksite located at 1237 S Rangline Rd, Joplin, MO 64801:  Employees were exposed to electrical shock hazards due to the electrical outlet box being plugged into another set of extension cords in series and not in accordance to the listing, labeling, and certification of the extension cords. On or about February 8, 2022, a homemade electrical outlet box was being used by employees to power tools and equipment. The outlet box was powered by two extension cords that were plugged together inseries and run back to the main powerbox.

At a worksite located at 1237 S Rangline Rd, Joplin, MO 64801: On or about February 8, 2022, a homemade electrical outlet box was being used by employees to power tools and equipment. Employees were exposed to electrical shock hazards and were not provided guards or other forms of enclosures to avoid accidental contact with live parts of electrical equipment.

Employees were exposed to fall hazards and were not provided with a guardrail system, safety net system, or personal fall arrest system to protect them from falling. Employees were exposed to a fall of 15 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 1237 S Rangline Rd, Joplin, MO 64801: On or about February 8, 2022, employees were using an extension ladder to gain access to an area of the building where they were installing decking. Employees were exposed to fall hazards and were not provided with a ladder that was positioned at least 3 feet above the upper landing being accessed. The ladder was only one foot above the lading and employees were exposed to a fall of 15 feet to the ground below.
