29 CFR  1926.453(b)(2)(v) :  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  Building #2 southwest entrance; On or about March 25, 2022, employee(s) utilized a Genie aerial lift without wearing fall protection to access a building roof top. This exposed the employees to a fall greater than six (6) feet to the concrete ground below.

29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  Building #2 southwest entrance;  On or about March 25, 2022, employees were performing roofing work activities from the roof of a building without the use of fall protection. This exposed the employees to a fall greater than six (6) feet to the concrete ground below.
