Suspension - On or about March 29, 2022,  employees were observed on the flatbed trailer without fall protection at a height of 56 inches, exposing employees to a fall hazard.  b. Press Area - On or about March 29, 2022, employees were assigned to get on top of a load of material on a flatbed in order to rig the gantry crane to move materials for the press. The employees were approximately 9 feet 5 inches off the floor and 5 feet to the inside of the trailer, both distances reflect fall hazards that the employees were exposed to.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a. Drop Down Trailer - On or about March 29, 2022, the flexible cord for the circular saw was damaged near the plug end which was plugged into an extension cord for the employee to cut the top of the deck to relieve bulging. The employees were exposed to an electrical hazard.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
