On or about March 22, 2022, two employees were securing shingles to a roof using pneumatic nail guns without the use of eye protection, exposing the two employees to struck by hazards.

29 CFR 1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. On or about March 22, 2022, two employees were using a ladder that did not extend 3 feet above the roofline while climbing to the roof to secure shingles to the roof, exposing the two employees to fall hazards.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section. On or about March 22, 2022, two employees were securing shingles to a roof under new construction at approximately 20 feet high without the use of fall protection, exposing the two employees to fall hazards.
