29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch.   On or about March 30, 2022, on the north side of the shop a Dayton Electrical bench grinder (motor reference # 60064RH75A2), used for the fabrication tasks, was used without the tongue guard.

29 CFR  1910.243(a)(1)(i): Portable power-driven circular saw(s) having a blade diameter greater than two inches were not equipped with guard(s) below the base plate or shoe which automatically and instantly returned to the covering position when the tool was withdrawn from the work.   On or about March 30, 2022, employees on the south side of the shop used a Dewalt Chop saw for metal wire rope cutting without proper guards.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located, and identified so that they were readily accessible without subjecting the employees to injuries.  On or about March 30, 2022, the fire extinguisher was blocked on the northeast side of the shop, which employees could not access.
