29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Juniper Park Limited Partnership, 2205 W 29th Ave, Denver, CO 80211:Between December 30, 2021 - March 18, 2022, while the facility was operating in outbreak status, the employer did not ensure that all employees who were required to wear tight-fitting respirators while conducting normal job operations were medically evaluated prior to use of the respirator.   Note: The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.

29 CFR  1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:   (a) Juniper Park Limited Partnership, 2205 W 29th Ave, Denver, CO 80211: Between December 30, 2021 and March 18, 2022 and on or before 3/29/22, the employer did not ensure that all employees who were required to wear a tight-fitting respirator while conducting normal job operations during "outbreak status" and when an employee entered an isolation room, were given a fit test prior to initial use of the respirator.
