29 CFR  1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) Jobsite- 1470 McFarland Road, Pittsburgh, PA: On or about March 30, 2022, employees working from a ladder jack scaffold were not wearing a personal fall arrest system and the employees were exposed to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Jobsite- 1470 McFarland Road, Pittsburgh, PA: On or about March 30, 2022, employees engaged in residential construction on the porch roof were exposed to fall hazards of approximately 11 feet from the eave of the porch roof to the ground below.
