The employer failed to ensure that employees who are operating an aerial lift at the jobsite be adequately protected from falling out the basket. An employee received fatal injuries after being ejected from the basket of an aerial lift at the jobsite when it tipped over.

The employer is failing to protect employees from fall hazards associated with operating aerial lifts. This was most recently observed on or about March 17, 2022 at the jobsite located at 3114 Pioneer Point Road, Reeds Spring, Missouri 65737. The employer failed to ensure that employees who are operating an aerial lift do not move the equipment when the basket is elevated in a working position. An employee received fatal injuries after being ejected from the basket of an aerial lift at the jobsite when it tipped over.
