29 CFR 1910.147(c)(4)(ii):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section:  Employees engaged in production activities at the Wellington Technologies facility located at 1119 W Scott Street, Beatrice Nebraska, were exposed to electrical hazards in that there were no documented lockout procedures used by employees to service equipment were specific for locking out equipment, including, but not limited to, the Mixer.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  Employees engaged in production activities at the Wellington Technologies facility located at 1119 W Scott Street, Beatrice Nebraska, were exposed to potential smoke-related hazards in that a door that was to be used as an exit was not marked with a sign reading "Exit".
