29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. On or about and prior to April 1, 2022, the employer failed to report the in-patient hospitalization of an over the road truck driver within twenty-four (24) hours of the occurrence related to a March 9, 2022 injury/accident at a workplace located at 9799 Heartland Court, Columbus, Ohio 43217. The accident victim succumbed to fatal injuries on  March 30, 2022.
