An employee, working from a scissor lift to work near metal rafters and crossbars, did not wear a hard hat to prevent head injury from impact.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) On or about 04/01/2022 at the site located at 3131 Transit Road. An employee, working from a scissor lift working on installing greenhouse components, did not secure the mid-chain of the basket to complete fall protection of the scaffold platform.  Employee was exposed to fall of 14 feet.   b) On or about 04/01/2022, at site located at 3131 Transit Road.  An employee, stood on the mid-rails and sat on top-rails of a scissor lift basket which was 14 feet above a lower level and was not protected from fall.    NO ABATEMENT CERTIFICATION REQUIRED
