29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about April 6, 2022, employees on the northside of the site were performing masonry work on a scaffold that was not fully planked, leaving a space greater than 1 inch between the platform (planks) and uprights.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffolds, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  On or about April 6, 2022, employees ascended vertical scaffold members of a tubular frame scaffold to perform masonry work at a height greater than two feet above a lower level without an appropriate means of access.

29 CFR  1926.451(h)(1):29 CFR 1926.451(h)(1):  Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects.  On or about April 6, 2022, on the north side of the site employees were performing masonry on a scaffold without hardhats to protect them from overhead falling objects.
