29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts, flying chips and sparks:  a) Jobsite located at 150 Commerce Drive, Aston, PA - The employer did not provide a method of machine guarding to protect employees from coming in contact with rotating spindle of  the Toshiba Machine Company table type horizontal boring and milling machine, Model # BFT-13CR3R4PD. An employee operating the metal boring machine came into contact with rotating spindle and attached metal parts sustaining fatal injuries.  Documented to occur on or about April 6, 2022.
