29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  At a commercial worksite located at 8695 NE Shoal Creek, Kansas City, Missouri. Employee(s) were exposed to injury from a fall hazard at a heights of 13  feet from the working level to the ground. Employees were conducting masonry work from a scaffold without proper access or egress. Employees used  crossbraces to climb to the working level.

At a commercial  worksite located at 8695 NE Shoal Creek, Kansas City, Missouri.  Employees engaged in masonry activities on a tubular framed scaffold were exposed to struck by injuries from falling tools and materials. Toe boards were not provided for additional protection.

Employees were performing masonry  operations at an elevated height on  roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.
