29 CFR  1926.25(a):  During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At the residential construction site located at Lot 79 Eagles Ridge, Blue Springs, MO:  Employees working throughout the worksite were exposed to slip and trip hazards from construction debris located around the base of a ladder used to access the roof of the structure.

The lanyard was not connected to the D ring on the harness at the center of the employee's back.
