29 CFR  1910.23(c)(11):  Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1);  (a)  On or about April 12, 2022; at a jobsite located at 309 Allegiance Ct, in Appleton, Wisconsin; on the west side of the building, the orange six step step ladder was used to gain access to an upper roof area did not extend at least three feet past the landing exposing employees to fall hazards.  (b)  On or about April 12, 2022; at a jobsite located at 309 Allegiance Ct, in Appleton, Wisconsin; on the east side of the building, the yellow six step step ladder was used to gain access to an upper roof area did not extend at least three feet past the landing exposing employees to fall hazards.

On or about April 12, 2022; at a jobsite located at 309 Allegiance Ct, in Appleton, Wisconsin; fall protection was not utilized while installing a replacement HVAC system on the top of a low slope commercial roof exposing employees to fall hazards of approximately 18 feet 10 inches.
