29 CFR  1910.178(l)(2)(ii):Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On or about April 14, 2022, at an establishment located at 840 South 62nd Avenue Wausau, WI 54401, the employer did not ensure that employees operating powered industrial vehicles had received formal instruction on site and truck specific topics prior to allowing them to operate the lifts.

On or about April 14, 2022, at an establishment located at 840 South 62nd Avenue Wausau, WI 54401, the employer has not performed an evaluation of operators performance every three years.
