29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about April 08, 2022, the employer did not develop and implement an energy control program consisting of energy control procedures for equipment and employee training to protect them while engaged in cleaning and maintenance activities.  Employees were exposed to crushing and amputation injuries from unexpended startup on the Jinan Sunward Machinery mixers (Model # SLHY 1.7) while they reached into the equipment for the purpose of cleaning the mixer tub and the auger.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
