29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Jobsite - On or about April 13, 2022, employees working in the approximate 100 foot by 3.5 foot trench, measured at between 5 feet and 6.5 feet deep, were not protected from cave-ins where an adequate protective system was not utilized.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. Jobsite - On or about April 13, 2022, the spoil piles excavated from the approximate 100 foot by 3.5 foot trench were not kept at least 2 feet back from the edge of the excavation at all locations along the trench. Employees working in the trench to install water lines were exposed to the hazard of material falling into the trench on them.
