29 CFR  1926.602(d):The employer did not ensure that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l).  Jobsite - On or about March 30, 2022, The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in 1910.178(l). An accident with a vehicle in the parking lot resulted from the inexperience of the untrained operator.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.   Jobsite - On or about April 11, 2022, debris, some with protruding nails, was not kept clear from the work area where an employee was working to re-frame a ground level window opening in close proximity to the debris.
