29 CFR 1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  On the roof at 7304 N Nebraska Avenue in Tampa, FL: On April 11, 2022, the employer exposed employees, cleaning skylights, to a fall hazard of 22 feet, in that employees were not protected from falling through the skylight by use of a guardrail, travel restraint system or personal fall arrest system.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a) On the roof at 7304 N Nebraska Avenue in Tampa, Fl: On April 11, 2022, the employer exposed employees to fall hazards, in the fall protection training was not provided to employees when they were required to use personal fall arrest system.
