29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. Somerset County Bridge C0705 - Old York Road at or near the intersection of Windmill Way, Branchburg, NJ  The employer did not ensure employees were protected from falls while working from bracket scaffolds.  Employees engaged in bridge building activities, placing concrete  for the abutment, worked from bracket scaffolding  at heights greater than 10 feet without guardrails and not utilizing any other means of fall protection.   Violation observed on or about April 18, 2022

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  a. Somerset County Bridge C0705 - Old York Road at or near the intersection of Windmill Way, Branchburg, NJ  The employer did not ensure that reinforcing steel was properly guarded.  Employees engaged in the placing of concrete for a bridge abutment worked in areas where the reinforcing steel (rebar) at the foundation extended more than 20 inches and was not protected with caps.  Violation observed on or about April 18, 2022

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Somerset County Bridge C0705 - Old York Road at or near the intersection of Windmill Way, Branchburg, NJ  The employer did not ensure portable ladders were extended at least 3 feet above the landing surface. Employees engaged in roadway bridge building activities used the upper half of a Werner extension ladder as a means of access/egress to the scaffold platform that did not extend 3 feet or more above the landing surface.    Violation observed on or about April 18, 2022

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:   a. Somerset County Bridge C0705 - Old York Road at or near the intersection of Windmill Way, Branchburg, NJ  The employer did not ensure that damaged ladders were marked "Do Not Use" or removed from service. Employees engaged in the construction of a bridge buttress used a Werner aluminum extension ladder (upper half) as a means of access to a scaffold platform where the ladder was damaged at the top and had bent rungs.  Violation observed on or about  April 18, 2022
