On and about 04/15/2022, employees used a drill press which did not have a guard to prevent an operator from being exposed to rotating edges of the drill bit and the chuck.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: 615 Arrow Lane, O'Fallon: Employer is exposing employees to hazardous conditions where the employees are permitted to use a pedestal wheel grinder which does not have a tongue guard. On and about 4/15/2022, employees used a pedestal bench grinder without its tongue guard on the left wheel. The employees were exposed to pinch points and potential broken pieces of the wheel.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective: 615 Arrow Lane, O'Fallon: Employer is exposing employees to hazardous conditions where the employees are permitted to a drill press which has an ungrounded attachment plug. On and about 04/15/2022, the ground pin of the attachment plug of the drill press used in the workplace was dislodged, making the path to ground non-continuous and exposing employees to electrical hazards.
