Location - 11 Carey Avenue, Burlington, MA.  On or about April 20, 2022, five employees were exposed from discarded lumber with protruding nails that had not been removed from the work area. This hazard existed on the right side of the house where employees would walk to gain access to the back of the house.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 11 Carey Avenue, Burlington, MA.  On or about April 20, 2022, the following fall hazard instances were observed;   Instance A: Five employees were exposed to fall hazards greater than fifteen feet above lower level from multiple window openings. The unprotected windows were located on the second floor. The employer/employees constructed the walls with the window openings in them. Employer had employees placed planking or railing system over the window openings during inspection.  Instance B: Five employees were exposed to fall hazards greater than sixteen feet above lower level. The employees were working/walking next to unprotected side/edges while framing. The employees were on the attic deck placing planks down.   Instance A:  An employee was exposed to fall hazard greater than fourteen feet above lower level from unprotected second floor deck. The employee assisted the owner with securing a plank.
