An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)	on or about 04/21/22, on the second floor balcony of 218-47 82nd Ave. Oakland Gardens, NY  Employees walking on an unprotected balcony to gain access to a roof were exposed to the hazard of falling approximately 12 ft. to the ground at the exterior of the single family house.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a)	on or about 04/21/22, on the second floor balcony of 218-47 82nd Ave. Oakland Gardens, NY  Employees walking on an unprotected pitch roof with ratio of 6 in 12 were exposed to the hazard of falling approximately 22 ft. to the ground.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  a)	on or about 04/21/22, on the ground of 218-47 82nd Ave. Oakland Gardens, NY  Employees were using a portable ladder to gain access to a balcony. The ladder side rails extended less than two foot above the upper landing surface. The employees were exposed to the hazard of falling approximately 12 ft. to the ground below.   b)	on or about 04/21/22, on the second floor balcony of 218-47 82nd Ave. Oakland Gardens, NY  Employees were using a portable ladder on the second balcony to gain access to a roof. The ladder side rails extended less than two foot above the upper landing surface. The employees were exposed to the hazard of falling approximately 22 ft. to the ground below.
