29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Joshua Enterprises Inc., dba Joshua Label on or about April 4, 2022, at the address of 12900-B Eckel Junction Road, Perrysburg, OH: The employer did not ensure that the three ceramic anilox rollers of the Allied Gear Flexomaster 7" 1B Press were adequately guarded.  Employees working on or near the Flexomaster were exposed to in going nip point hazards between the ceramic anilox roller and printing cylinder.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Joshua Enterprises Inc., dba Joshua Label on or about April 4, 2022, at the address of 12900-B Eckel Junction Road, Perrysburg, OH: The employer failed to report the amputation of an employee that was a result of a workplace injury/incident.  The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employee's hospitalization within 24 hours of receiving amputation injuries.  The employer later contacted the OSHA Office on April 19, 2022.
