29 CFR  1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:  (a) 17590 Kurtz School Road Stewartstown PA - An employee worked from a pump jacks scaffolding to hang siding on the residential home.  The scaffolding consisted of an Alum-A-Pole pump jack scaffolding, which included two poles, two pump jacks, and a stage, erected across the side of the residential home,  The employer attached a 2x12 plank that extended it from 3 to 4 feet past the end of the engineered metal planking and was working from it, thereby exposing the employee who worked on the scaffolding to the hazard of the 2x12 snapping and the employee falling to a lower level, on or about April 25, 2022.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.451(b)(8):At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over:  (a) 17590 Kurtz School Road Stewartstown PA - An employee worked from a pump jacks scaffolding to hang siding on the residential home.  The scaffolding consisted of an Alum-A-Pole pump jack scaffolding, which included two poles, two pump jacks, and a stage, erected across the side of the residential home,  The employer attached a 2x12 plank that extended it from 3 to 4 feet past the end of the engineered metal planking and was working from it and would use it to step around to get to the other pump jack scaffold to exit the platforms , exposing the employee who worked on the scaffolding to the hazard of the 2x12 snapping because it was not on a bearer to support the 2x12 and the employee falling approximately 25 feet to a lower level, on or about April 25, 2022.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
