29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  A) 90 Stults Road, Dayton NJ  The employer failed to certify that each operator had been evaluated on the forklift.  The employer failed to provide documentation, which included the name of the operator, the date of the evaluation and the identity of the person performing the evaluation.  Exposure occurred on or about 4/26/22.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  A) 90 Stults Road,  Dayton, NJ  The electrical equipment on the back wall of the warehouse did not have free and clear access. The panel was blocked by the PIT battery charging stations that was placed in front the electrical equipment.  Hazard observed on or about 4/26/22
