A) On or about April 25, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415  The platform of a suspended scaffold used by employees while performing exterior brick work (i.e. brick grinding, brick-pointing, lintel work) on the 3rd and 4th floors of a residential apartment building was observed missing a guardrail system along all open sides and platform ends exposing employees to fall hazards.  B) On or about April 26, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415  The platform of a suspended scaffold accessed by an employee on April 26, 2022 was observed missing a guardrail system along all open sides and platforms ends resulting in an employee falling approximately twenty (20) feet to the lower level on April 26, 2022. 29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  A) On or about April 26, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415  The employer failed to ensure each employee required to access a two (2) suspended scaffold while brick-pointing and window caulking was properly trained in the recognition of fall hazards resulting in a workplace fatality on Tuesday, April 26, 2022 in which an employee fell approximately twenty (20) feet to the lower level.  ABATEMENT CERTIFICATION IS NOT REQUIRED
