29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   a) On April 21, 2022, and times prior the employer did not have an effective system in place to identify and correct hazards to which their employees were exposed. These include various fall hazards such as using concrete form panels as ladders, fall protection not used when working on the upper deck, a stair or ramp was not used where the break in elevation was greater than 19 inches, and working from scaffold planks that were six inches wide.       Note) Certification of corrective action is required for this item.
