Ipswich, Ma  On or about 4/29/2022, the employer did not ensure employees stripping a residential dwellings roof, were protected from falls to a lower level.

Ipswich, Ma  On or about 4/29/2022, The ladder used to access the roof did not extend 3 Ft. above the roof edge.
