29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about May 2, 2022, during the installation of roofing material at the job site located at 6811 Parma Park Boulevard, Parma Heights, OH 44130; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of an eye injury form impact, or from flying objects while utilizing a pneumatic nail gun.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about May 2, 2022, during the installation of roofing material at the job site located at 6811 Parma Park Boulevard, Parma Heights, OH 44130; the employer did not ensure employees working on the roof of a single family home were protected from a fall of approximately 24 feet from an 8:12 pitch roof  by ensuring the utilization of  personal fall arrest systems/devices.
